Information provided is CONFIDENTIAL and will provide the reviewing practitioner key information to ensure the safe prescribing of HRT.

In order to review your HRT medication, we need an accurate measurement of your height, weight, and blood pressure.

We cannot process this form without a blood pressure reading. If you don't have a BP machine at home, please use the Health Station in our waiting room. You can also measure your BP in some pharmacies.

Risks and Side Effects

Like any medicine, HRT can cause side effects but it's common to have no side effects or only mild ones.

Side effects can include: headaches, breast tenderness, unexpected vaginal bleeding, nausea, mood changes, leg cramps, mild rash or itching, diarrhoea, hair loss or growth, acne, and more. 

While HRT can be effective at relieving menopause symptoms, it can also increase your risk of developing certain conditions including: osteoporosis, loss of muscle strength, breast cancer, blood clots, or stroke.
